Schedule

Book online or call to schedule. Bring your driver's license, glasses or hearing aids if you use them, and a list of medications.

Exam

A certified medical examiner performs the full DOT physical including vision, hearing, blood pressure, and urinalysis.

Certificate

If cleared, you walk out with your medical examiner certificate the same day. We submit to the FMCSA registry on your behalf.

You're welcome to download the MCSA-5875 (the Long Form) ahead of your appointment to review or fill out at home, or just fill it out when you arrive.

The MCSA-5875 is the official DOT Medical Examination Report Form. It covers your medical history and the information our provider needs to complete your physical exam.

Bring your driver's license, your glasses or hearing aids if you use them, and a list of any medications you're currently taking. Our provider will go over the form with you and complete the physical exam when you arrive.

Onsite Service

We come to your operation.

Employers in trucking, ag, and construction know that pulling people off the job for physicals costs real money. For 6 or more exams, we'll come to your site and run them back-to-back so your team stays moving.

$120 per exam, with an optional urine DOT drug screen for $75 per exam. Most onsite days run two to three hours depending on group size.
